Thank you for submitting your Community Partner Request for Proposal Letter of Intent 2016. You may now download the application materials from our website. To complete your request for funding, submit one electronic copy and four paper copies of the following by 4:00 p.m. on Monday, March 14, 2016.

Additionally, one current copy of each of the following must be on file with United Way of Adams County:

  • Confirmation letter of non-profit status (501 C 3)
  • Organization By-laws
  • Organization Personnel Policies and Procedures
  • List of current Board of Directors
  • List of Board Meeting Dates and Locations for 2015 and 2016 calendar years
  • Statement of policy of non-discrimination
  • Statement of current litigation status
  • Most recent 990
  • Current Organizational Budget
  • Most recent audit or CPA signed financial review
